- [ ] Identify the exact source version and the task under review.
- [ ] State the expected behavior and the conditions that matter.
- [ ] Follow the change through its callers, stored results and user interface.
- [ ] Examine a relevant failure, interruption or recovery path.
- [ ] Check that the tests can detect the failure they claim to catch.
- [ ] Challenge each finding with its mechanism and counterevidence.
- [ ] Verify the correction against the intended behavior and likely regressions.
- [ ] Record the decision, supporting evidence and remaining limits.
